• Driver’s License
• Social Security Card or Number
• Your Employer Information (Name and Address)
• Advanced Directives/Living Will
• List of all Medications and Dosages
• All Completed Registration Forms

If your insurance plan requires a referral from your primary care physician for outpatient surgery, please be sure the referral is sent to our office prior to your visit.
